Title: Coordinator - Healthy Community Zones

$72,509 - $104,330 / Posted Thru: 5-12-24

Location: Remote/Virtual - Must Live in Michigan - In-person Meetings Required

PURPOSE:This high-level position will serve as the statewide coordinator for Healthy Community Zones, a new initiative in three communities that are funded by Michigan Department of Health and Human Services (MDHHS). This position will be responsible for providing and/or procuring expert advice, consultation and technical assistance and oversight to funded community-based agencies to ensure they have the capacity to engage key partners in planning and implementing priority strategies known to increase food security, improve physical activity, implement approaches that promote optimal childhood nutrition, create and deploy a state-wide multimedia campaign to increase awareness of how to live a healthy lifestyle and develop a taskforce/workgroup with state agencies and other state-level decision makers to change policies to support locally determined environmental changes and increase access to other Social Determinants of Health.

Critical project management responsibilities include providing HCZ agencies technical assistance on program development, implementation, evaluation, and fiscal management as needed. In addition, this position has lead responsibility for the coordination of needed program-specific technical assistance from the Cardiovascular Health, Nutrition, and Physical Activity (CVHNPA) Section. Position responsibilities also entail the development and management/maintenance of complex organizational partnerships with state and local coalitions and organizations as needed to establish, maintain, and evaluate the HCZ approaches to support healthy lifestyles and prevent chronic diseases to reduce health disparities.

DUTIES AND RESPONSIBILITIES:

Serve as the lead liaison between the HCZ contract agencies and the MDHHS Cardiovascular Health, Nutrition and Physical Activity (CVHNPA) Section to implement evidence-based programs and strategies.

  • Coordinate implementation of evidence-based strategies to improve population health.
  • Coordinate communication and technical assistance between MDHHS, the HCZs and state and community partners to ensure all strategies and activities are fulfilled at the community level.
  • Coordinate HCZ staff trainings, as needed, to meet program requirements. Arrange and participate in joint learning and sharing opportunities for HCZ staff and partners.
  • Convene meetings of HCZ agency staff, MDHHS lead staff, and key state and community partners as needed.
  • Lead communication and presentations to MDHHS senior leadership and partners.
  • Work with program consultants, other internal team members, and HCZ's as necessary, to develop media releases highlighting successes of MDHHS and the HCZs in implementation of project strategies, for use by MDHHS, the HCZs and other various statewide and community groups.
  • Develop and manage a state-wide multimedia public awareness campaign to reduce obesity and promote healthy lifestyles.

Working with the MDHHS evaluation consultant and HCZ staff, develop and provide coordinated technical assistance on program development/implementation. Support the HCZs through coordinating technical assistance in the following areas:

  • Core project management and leadership of the local project.
  • Partnerships and subcontracts.
  • Financial management and accountability.
  • Compliance with established grant work plan and timelines.
  • Evaluation data collection and reporting.

Work with the MDHHS evaluation consultant to facilitate monitoring HCZ performance measures and oversight of required data submission from HCZs. Coordinate and ensure all reports required are submitted. Work with the MDHHS Evaluation Lead and HCZ staff to:

  • Implement common data collection instruments, as necessary and appropriate.
  • Ensure the submission of required evaluation reports.

In collaboration with program consultants and analysts, coordinate communication and activities for managing contracts with CDCN lead agencies to ensure all required grant strategies are being fulfilled at the community level:

  • Coordinate the preparation and release of proposals and program agreements process.
  • Coordinate and conduct HCZ agency site visits for purposes of fiscal management and contract management adherence.
  • Provide/coordinate feedback on progress reports.
  • Develop and disseminate program materials and summaries to support program areas.

Participate in and/or assists with the implementation of new programs and projects.

Maintain positive, strong, credible professional and interpersonal relationships with all parties and always represents the best interest of MDHHS and MPHI.

QUALIFICATIONS/REQUIREMENTS:

Education: Possession of a master's degree in public health or related field such as communication, social work, health communication, or specific master's degrees related to health promotion or health professions, including physical activity and nutrition.

Experience: Four years' experience in public health, health systems, or with community health agencies/non-profit agencies. Extensive knowledge and experience in addressing health disparities, inequities, and the social determinants of health.

Important Skills and Characteristics:

  • Ability to accept responsibility with minimum day to day oversight.
  • Excellent verbal, interpersonal, and problem-solving skills.
  • Grant-writing expertise and the ability to maintain relationships with multiple partners in variety of locations and professions.
  • Advanced level skills working with multiple external partners as well as with internal chronic disease staff are necessary.
  • Knowledge of chronic disease control and prevention programming, evaluation design and report writing, and a sound understanding of prevention and control strategies are desirable.
  • Knowledge specific to implementing prevention strategies for obesity and chronic disease in Michigan is desirable.
  • Familiarity with state government, state procedures for contracting and reporting is preferred.
  • Computer literacy in Microsoft Office programs is expected.

Work Environment and Physical Requirements: No significant physical activities are required in this position other than deskwork and lifting of office materials and equipment and materials <50#. Involves use of standard office equipment and significant computer related activities such as viewing a CRT or VDT screen at least 85% of the time. May require a valid vehicle operator's license where needed to perform the duties of the position.

RESPONSIBILITY FOR THE WORK OF OTHERS: No assigned responsibilities.

The coordinator will function independently regarding: communication and technical assistance to partners and funded agencies; development of problem solving approaches; budget and work plan review and recommendations for approval and modification; trouble shooting project and contract management activities; documentation and maintenance of relevant reports, correspondence, and contracts; meetings, appointments, and travel arrangements/schedule; collaboration and communication with multiple, diverse stakeholders.

IMPACT ON PROJECTS, SERVICES AND OPERATIONS: This individual will be directly responsible for the completion of tasks related to this project. The completion of these tasks will enable the program to fulfill data related goals, as well as general program requirements. This position will strengthen the ability of the program to provide data support and produce quality reports from data reviews.
